About This Settlement

Plaintiffs in Concepcion, et al. v. Deanco Healthcare d/b/a Mission Community Hospital (Case No. 23STCV29292) alleged that Deanco Healthcare failed to adequately protect patient information from a data breach that allegedly occurred on May 1, 2023, which may have compromised sensitive personal information including Social Security numbers, financial account information, and health insurance data. Deanco Healthcare has not admitted any wrongdoing but has agreed to a $1,546,409.42 class action settlement to resolve these allegations. Who Qualifies

• Your personally identifiable information may have been compromised in the Deanco Healthcare data breach that allegedly occurred on May 1, 2023
• You were a patient or otherwise had personal data on file with Deanco Healthcare (doing business as Mission Community Hospital) at or around the time of the alleged breach
• California residents affected by the breach may also qualify for an additional $100 statutory payment
• You must submit a valid claim form by the August 12, 2026 deadline
• Documentation of any out-of-pocket losses may be required to support claims for reimbursement

What You May Receive

Eligible class members may receive one or more of the following benefits: (1) up to $5,000 in reimbursement for documented out-of-pocket losses that can be reasonably attributed to the alleged data breach, such as unreimbursed bank fees, credit monitoring costs, or lost time; (2) an estimated pro rata cash payment based on the remaining settlement fund after other costs and payments are deducted — the actual amount will depend on the total number of valid claims filed; (3) an additional $100 statutory payment for eligible California residents; and (4) two years of medical monitoring services, including credit monitoring, identity theft insurance, and dark web monitoring, at no cost. Individual payout amounts are not guaranteed and will vary based on the type of claim submitted and the number of valid claims received.

How to File a Claim

1. Visit the official settlement website at DeancoDataBreachSettlement.com to access the claim form and review full eligibility details
2. Complete the claim form accurately, providing your personal information and selecting the type of benefit(s) you are claiming (documented losses, pro rata payment, California statutory payment, and/or monitoring services)
3. Gather and attach any required supporting documentation for documented loss claims — acceptable documents may include credit card statements, bank statements, invoices, telephone records, and receipts that support your claimed out-of-pocket losses
4. Submit your completed claim form with all required documentation by the claim deadline of August 12, 2026 — late submissions may not be accepted
